Order a death certificate

Only deaths that have occurred in Yukon are registered in Yukon Vital Statistics.

Eligibility

A death certificate can be issued to any person who requires it. The registrar will decide if your reason is valid. Certificates can also be issued to a person on a court order.

Order a death certificate

1. Complete the application.
2. Submit your application and payment of $10.
   In person: At any Vital Statistics office or Territorial Agent. Our office at 204 Lambert Street, Financial Plaza, 4th floor in Whitehorse is open Monday to Friday from 9 a.m. to 5 p.m. We accept cash, cheque, money order, VISA, Mastercard and American Express.
