Getting started: What is pre-diabetes
A registered nurse leads this program and will help you understand:
- pre-diabetes;
- diabetes;
- risk factors;
- diagnosis; and
- the important management strategies for a healthy life.
Diabetes Wellness Program
This is a 2-part workshop where you will:
- connect with others living with diabetes;
- take practical steps to eat healthy, manage medication, and stay active; and
- learn from a variety of diabetes specialist such as:
- dietitians;
- nurses;
- pharmacists; and
- exercise specialists.
Diabetes management- Insulin 101
This is a 2-hour session to help you understand insulin. We recommend this program for people who:
- already know about diabetes; or
- have attended the Diabetes Wellness Program.
In this session, you’ll:
- get expert guidance on insulin use;
- connect with peers; and
- increase your confidence in insulin use.
Next level ‘Carb Counting’ for diabetes management
A registered dietitian and certified diabetes educator lead this workshop. It focuses on managing carbohydrates intake for better blood sugar control.
We recommend this program for people who:
- already know about diabetes; or
- have attended the Diabetes Wellness Program.
Your health your way
This is a 6-week online program. We created this program in partnership with Self-Management British Columbia. It aims to help people who have ongoing or are at risk of developing chronic condition:
- better manage your symptoms;
- learn self-management skills; and
- reach goals by taking small steps and discover tips to manage day-to-day activities.
You’ll also get support from people with similar health concerns.
Making sense of pain
A self-management program developed in collaboration with PainBC. We designed this program for people living with chronic pain. This 10-week workshop will provide participants opportunities to learn about pain through:
- the various factors that can cause or make pain worse; and
- simple strategies that can help participants better manage pain in everyday life.
How to join the programs
Eligibility criteria
- Any resident of Yukon who has a chronic health condition can register for any of these programs.
- Anyone interested in improving their overall health is also eligible.
