The home health monitoring service is available in all Yukon communities. It's for Yukoners with chronic health conditions like diabetes, chronic obstructive pulmonary disease (COPD) and high blood pressure.
You’ll receive an easy-to-use tablet with health monitoring equipment you can use at home. Other kits you may get include:
- blood pressure machine;
- weight scale;
- blood oxygen sensor; and
- thermometer.
Home Health Monitoring will enable you to:
- improve your understanding of your health condition;
- understand its impact on your daily activities; and
- learn how to effectively manage your symptoms.
This service will:
- provide you with greater confidence and security to live on your own;
- give peace of mind to your caregivers; and
- keep your primary care providers updated with your health status.
This service is not:
- intended to replace or replicate primary care services; or
- an emergency response system.
How to enroll in home health monitoring
There are two ways to enroll in home health monitoring:
Self-referral
- Contact or visit the Chronic Conditions Support Program (CCSP) office.
- Attend an intake appointment either online or at the CCSP office.
- Talk about your health goals with the CCSP team.
- With the CCSP team's assistance, decide whether home health monitoring is suitable for you and what it will involve.
Physician referral
- Visit your family clinic or any health care provider.
- Ask your provider for a referral.
- Contact or visit the Chronic Conditions Support Program (CCSP) office.
- Attend an intake appointment either online or at the CCSP office.
- Talk about your health goals with the CCSP team.
- With the CCSP team's assistance, decide whether home health monitoring is suitable for you and what it will involve.
The Chronic Conditions Support Program will notify your primary care provider when you self-refer for this service.
