- palliative;
- rehabilitation services; and
- respite care.
Services are based on an individual’s assessed needs.

Community services include:
- care coordination;
- regional home support workers;
- community-based nursing; and
- regional therapy services (physiotherapy, occupational therapy or speech-language pathology).
Eligibility to access this service
Yukoners of all ages are eligible if they:
- have an assessed need for home care; and
- have, or have applied for, the Yukon Health Care Insurance Plan.
