How to fill out the form

The information you provide on the form must be legible.

Section 1: Name of extra-territorial corporation

Include the name of the extra-territorial corporation.

Section 2: Yukon registry number

Include the extra-territorial corporation’s Yukon registry number.

Section 3: Attorney for service

Include the name of the attorney for service and the effective date of their appointment. Their address must be in Yukon.

“Delivery address” refers to a physical location. Include an address or other description to identify a person’s home or place of business. This must not include a post office box.

“Mailing address” refers to where you send mail. Include a complete mailing address, including the postal code. This address can be a person’s home, place of business or post office box.

Section 4: Signature and date

The attorney signs and dates the form here.

Section 5: Alternative attorney for service

If you have an alternative attorney for service, put their name here. Also include their address and effective date of appointment. Their address must be in Yukon.

Section 6: Alternative attorney signature and date

The alternative attorney signs and dates the form here.

Section 7: Name of individual signing for extra-territorial corporation

Include the name of the individual signing for the extra-territorial corporation.

Section 8: Title of individual signing for extra-territorial corporation

Include the title of the individual signing for the extra-territorial corporation.

Section 9: Revoke previous attorneys

The individual signing for the extra-territorial corporation signs and dates the form here. This revokes any appointments of attorneys or alternative attorneys before this date.

Section 10: Date of signature

Include the date the form was signed.

After the form is completed, submit it. There is no cost for filing an appointment of attorney for service and alternative attorney.
